Physician Liaison – Value-Based Care (1099 Commission-Based)
We are seeking a highly motivated Physician Liaison to join our dynamic Value-Based Care team. Role Overview: The Physician Liaison will strategically build and sustain partnerships with healthcare providers, ensuring alignment with the principles and objectives of value-based care. This role requires a deep understanding of clinical operations, reimbursement models, and population health strategies to effectively communicate and promote collaboration among physicians, healthcare teams, and our organization.
Essential Responsibilities:
Develop and execute targeted outreach and engagement strategies to physicians and healthcare providers.
Educate and advise physicians and healthcare practices on value-based care models, quality measures, and outcome-based reimbursement structures.
Conduct regular provider visits, presentations, and meetings to strengthen relationships and encourage collaboration.
Identify and assess physician and practice needs to effectively align services and support value-based initiatives.
Monitor, analyze, and report on market trends, provider feedback, and performance metrics to refine engagement strategies.
Act as the key liaison between providers and internal teams to facilitate seamless communication and integration of value-based care programs.
Ensure timely and accurate documentation of provider interactions, outcomes, and follow-ups.
Minimum Qualifications:
Minimum of 3 years’ experience in healthcare sales, physician liaison, or clinical outreach roles.
Strong understanding of value-based healthcare models, reimbursement, and quality improvement initiatives.
Excellent interpersonal, communication, and presentation skills.
Demonstrated ability to build and maintain effective professional relationships.
Proven ability to work independently, manage time effectively, and achieve targeted outcomes.
Proficiency in standard computer applications, including CRM tools, data analysis, and reporting software.
Valid driver’s license, reliable transportation, and acceptable driving record.
Preferred Qualifications:
Clinical background or equivalent clinical experience highly desirable but not required.
Established relationships with local physician groups, healthcare providers, and healthcare administrators.
Physical Requirements:
Ability to travel regularly within assigned territories.
Frequent sitting, standing, and driving.

We are seeking a dedicated and compassionate Care Coordinator to specialize in Chronic Care Management. This critical role is open to Registered Nurses (RN), Licensed Practical Nurses (LPN), or Certified Medical Assistants (CMA). The ideal candidate will have a passion for supporting patients with chronic illnesses, improving care outcomes, and enhancing patient quality of life through proactive, coordinated care.
Position Summary: The Chronic Care Management Coordinator collaborates closely with patients, healthcare providers, and interdisciplinary teams to manage chronic conditions effectively. This role involves patient education, care plan implementation, ongoing monitoring, and ensuring adherence to treatment plans, ultimately reducing hospitalizations and optimizing overall patient health.
Key Responsibilities:
Chronic Care Coordination:
Develop individualized care plans focusing on chronic disease management and preventative care.
Coordinate regular patient follow-ups, medication management, and health assessments.
Patient Education and Support:
Provide education and counseling to patients and their families about chronic conditions, treatment options, and lifestyle modifications.
Serve as the primary point of contact to answer patient questions and provide ongoing support.
Collaboration and Communication:
Work closely with physicians, specialists, and interdisciplinary care teams to ensure comprehensive, patient-centered care.
Facilitate seamless transitions between different healthcare providers and care settings.
Monitoring and Evaluation:

Track and document patient interactions, health outcomes, and compliance with care plans using electronic health records (EHR).
Community and Resource Coordination:
Assist patients in accessing community resources, support services, and healthcare programs.

Qualifications:
Required:
Active licensure/certification as an RN, LPN, or Medical Assistant.
At least 2 years of clinical or care coordination experience, with an emphasis on chronic disease management preferred.
Skills:
Strong organizational, communication, and interpersonal skills.
Knowledge of chronic disease management protocols and preventative healthcare.
Ability to work independently and collaboratively within multidisciplinary teams.
Proficiency in electronic health records (EHR) systems and care coordination software.
